  • Background :

Previous research has shown that supplementation with fish oil had a protective effect on bronchoconstriction induced by exercise (EIB) in elite athletes, which could be put down to its anti-inflammatory properties.
Since IBE in asthma involves salting of proinflammatory mediators, it is possible that supplementation with fish oil may reduce the severity of EIB in asthmatic subjects.
- Objective :

Determine the efficacy of fish oil supplementation on the severity of EIB in asthmatic subjects.
- Study design :

Cross, randomized, double-blind.
- Part of the study :

Pulmonary function and exercise testing in a university research laboratory.
- Patients and measurements :

16 asthmatic patients with documented IBE were included in the study, first with a normal diet and with fish oil capsules containing 3.2 g eicosapentaenoic acid and 2.0 g of docohexaenoïque acid (oil diet fish, n = 8) or placebo capsules (placebo diet, n = 8), this daily for three weeks.
At the beginning of the study (normal operation) and at the end of each treatment phase was measured (in pre-exercise and post-exercise):
pulmonary function,
differential cell count percentage of induced sputum
= leukotriene metabolites concentrations C4 (LTC4), leukotriene E4 (LTE4), prostaglandin D2 (PGD2), cytokines (interleukin IL1béta and TNF-alpha,
metabolites eicosanoids leukotriene B4 (LTB4) and B5 (LTB5) from poly-morphonucléaires leukocytes (PMNLs).
- Results :

Under normal diet and placebo, subjects were presented IBE.
The fish oil diet improved pulmonary function to below the diagnostic threshold IBE with a reduction in the consumption of bronchodilators.
The percentage of induced sputum cell counts on and concentrations of LTC 4-LTE 4, PGD 2, IL-1beta and TNF-alpha were significantly reduced before and after exercise for the fish oil diet compared to normal diet and placebo diets.
There was a significant reduction of LTB4 and a significant increase in production by PMNLs LTB5 enabled for the fish oil diet compared to normal diet and placebo diets.
- Conclusion :

Our data suggest that fish oil supplementation may represent a potentially beneficial nonpharmacologic intervention for asthmatic subjects with IBE.

Should we change our diet and / or our asthmatic patients? If so, should we do it sustainably? And what specific recommendations?

We're not there yet: some studies show results more or less significant in terms of asthma and allergies.

In 2002, an Australian study had objectified improved asthma control.

In 2004, a Canadian study was interested in a broader impact by supplementing pregnant women in hopes of reducing the risk of allergy for their future children; the results were inconclusive.

In total, we still have to wait some time before we have enough consistent evidence leading us to change our practices.
